  • Confidence with movement

All exercise programs are evidence-based and delivered by qualified health professionals.

D904 Referral Requirement

To access Exercise Physiology services through DVA, a D904 referral is required.

Key points:

  • The D904 referral must be completed by your GP or medical practitioner
  • The referral authorises a cycle of care of up to 12 Exercise Physiology sessions
  • Services must align with the goals and conditions listed on the referral

If you are unsure whether your referral is current or suitable, our team can assist.

12-Session Cycle of Care

DVA Exercise Physiology services are delivered in a 12-session cycle of care.

During this cycle:

  • Your Exercise Physiologist provides structured, supervised exercise
  • Sessions may be completed 1 on 1 or in a group setting, depending on your needs
  • Exercises are progressed appropriately across the cycle

At the completion of the 12 sessions, a formal review is completed to assess progress, outcomes, and ongoing needs.

Reviews After 12 Sessions

A review is completed at the end of each 12-session cycle to:

  • Assess improvements in strength, mobility, balance, and function
  • Review progress toward health and functional goals
  • Determine the effectiveness of the current program
  • Provide recommendations for ongoing care

If further Exercise Physiology is required, a new D904 referral may be needed to commence another cycle of care.

1 on 1 DVA Exercise Physiology Sessions

1 on 1 sessions provide individualised support and close supervision.

These sessions are ideal for veterans who:

  • Have complex or multiple health conditions
  • Require individual pacing and progression
  • Are new to exercise
  • Need additional confidence and support
  • Have specific functional goals

Your Exercise Physiologist focuses entirely on your needs during each session.

DVA Group Exercise Physiology Sessions

Where appropriate, Exercise Physiology services may also be delivered in small supervised group sessions.

Group sessions:

  • Are fully supervised by Accredited Exercise Physiologists
  • Remain individually tailored within the group
  • Provide structure, routine, and social connection
  • Support ongoing exercise participation

Group exercise can be an effective and supportive option within a DVA cycle of care.
