To refer to our NDIS services?

If you want to refer a participant to our NDIS services there are a few options.

Option 1: Call Coordinated Fitness on 0417 765 818 and book an initial appointment with our Exercise Physiologists.

Option 2: Email admin on admin@coordinatedfitness.com and ask questions or book an initial appointment.

Option 3: Have your support coordinator call or email Coordinated Fitness on your behalf to book your initial

Option 4: Fill out our intake form below and it will notify our admin team you wish to make an appointment. It is always good to follow up with an email to admin@coordinatedfitness.com to ensure we received your form.

Can I self-refer for services or does my support coordinator required to send the referral?

Yes, you can self-refer to Coordinated Fitness Exercise Physiology services. However, if you would prefer your support coordinator to complete the paperwork or refer, they can as well. The choice is yours we are just here to help you out along the way.

How do I start my program with an Exercise Physiologist?

Once you have made your initial appointment you can start your treatment with your Exercise Physiologist. In your initial consultation we will build out your plan and help you along the way.

An Enhanced Primary Care Plan (EPC) is now called a Chronic Disease Management Plan?

Did you know EPC’s are now called Chronic Disease Management Plans. They are essentially the same thing just with an updated name. So you may hear people or doctors talking about a Chronic Disease Management Plan now instead of EPC’s.

Can I use a Chronic Disease Management Plan?

Yes, you can use a Chronic Disease Management Plan at Coordinated Fitness. If you wish to claim your rebate from Medicare you need to have your referral with you at your initial consult. We can also complete your rebate on the spot at your appointment if you have the correct referral. You will need a debit card for your rebate to go on to. You can pay with a credit card but the rebate must be onto a debit card.

Is there an out of pocket cost using my Chronic Disease Management plan?

Yes, there is an out of pocket expense. The rebate amount for these sessions is calculated on a 20minute consult. Our assessments and sessions take 45mins. We also write a report at your initial consult and final consult for your referring doctor. Therefore, there are our normal charges for your consult, and we do not bulk bill.

Can I use my health insurance for Exercise Physiology therapy?

Yes, Exercise Physiology is covered under several health insurance plan. We encourage you to call your health fund first to ensure your visit will receive a rebate.

How can I claim with private health insurance?

We have our Tyro system at Coordinated Fitness which can link to your health fund for your rebate. There are a few health funds which do not work with Tyro. If this is the case we will send you the receipt to manually process the refund on your end.

Can I use my health insurance for Group Exercise Physiology therapy?

Yes, Group Exercise Physiology is covered under several health insurance plan. We encourage you to call your health fund first to ensure your visit will receive a rebate.

If you choose to pay via a 10 pass for your group sessions, we will print out your receipt with our attendance for your sessions at the end of your pass. You will then need to submit both your receipt and attendance record to your health fund for your rebate.
