Coordinated Fitness provides Exercise Physiology services under Chronic Disease Management (CDM) plans to support people living with long-term health conditions. These services are delivered by Accredited Exercise Physiologists and focus on improving strength, mobility, function, and confidence through safe, evidence-based exercise.

A referral from your GP is required, and eligible sessions attract a Medicare rebate.

What Is a Chronic Disease Management Plan?

A Chronic Disease Management (CDM) plan is a Medicare initiative designed to support people living with chronic or long-term health conditions that have been present (or are expected to be present) for six months or more.

Under a CDM plan, eligible patients may access rebated allied-health services, including Exercise Physiology, to help manage their condition and improve overall health and function.

Who Is Eligible for a CDM Plan?

You may be eligible for a CDM plan if you are living with a chronic condition such as:

  • You must have a current referral from your GP
  • The referral must specifically include Exercise Physiology
  • Referrals are typically valid for a set number of sessions

Medicare Rebates

Exercise Physiology sessions under a CDM plan attract a Medicare rebate through Medicare.

Please note:

  • Medicare provides a partial rebate, not full coverage
  • Rebates are processed after your appointment
  • The rebate amount is set by Medicare

Our admin team can provide current rebate information and assist with claims.

We Do Not Bulk Bill

At Coordinated Fitness, CDM Exercise Physiology sessions are not bulk billed.

This means:

  • Payment is required at the time of your appointment
  • You will receive a Medicare rebate back after your session
  • There will be an out-of-pocket cost

This approach allows us to continue providing high-quality, individualised care with appropriate session lengths, regular reviews, and professional supervision.
