The Type 2 Diabetes Exercise Program at Coordinated Fitness is a structured, evidence-based program designed to support people living with Type 2 diabetes to improve blood glucose management, strength, fitness, and confidence with exercise. The program includes an individual assessment, followed by 8 fully supervised group exercise sessions, and a final review with feedback provided to your GP.

All sessions are delivered by Accredited Exercise Physiologists and attract a Medicare rebate.

What Is the Type 2 Diabetes Exercise Program?

This program uses clinically prescribed exercise to help manage Type 2 diabetes safely and effectively. Exercise plays an important role in improving insulin sensitivity, supporting blood glucose control, and reducing the risk of diabetes-related complications.

What's Included in the Program

Individual Initial Assessment

Your program begins with a 1 on 1 assessment with an Exercise Physiologist to:

• Review your medical history and diabetes management
• Assess strength, mobility, balance, and fitness
• Discuss goals and any barriers to exercise
• Ensure exercise is appropriate and safe

This assessment allows your Exercise Physiologist to tailor the group exercise sessions to your needs.

8 Supervised Group Exercise Sessions

Following your assessment, you will complete 8 group exercise sessions.

These sessions:

• Are fully supervised
• Are delivered by Accredited Exercise Physiologists
• Include individualised exercise prescription within the group
• Focus on strength, cardiovascular fitness, and functional movement
• Are progressed safely based on your response and confidence

Group sessions also provide motivation, routine, and social support.

End-of-Program Review and GP Update

At the completion of the program, a review appointment is completed to:

  • Assess improvements in strength, fitness, and function
  • Review progress toward your goals
  • Discuss ongoing exercise recommendations
  • Provide an update to your GP outlining outcomes and future recommendations

This ensures continuity of care and clear communication with your medical team.

Fully Supervised by Accredited Exercise Physiologists

All sessions in the Type 2 Diabetes Exercise Program are supervised by university-qualified Exercise Physiologists accredited with Exercise & Sports Science Australia.

This ensures:

  • Exercise is clinically appropriate
  • Technique and intensity are monitored
  • Safety is prioritised at all times
  • Programs follow current evidence-based guidelines

You are never left to exercise unsupervised.

Medicare Rebates Available

This program attracts a Medicare rebate through Medicare when you have a valid referral from your GP.

Please note:

  • Medicare provides a partial rebate
  • Sessions are not bulk billed
  • An out-of-pocket cost applies
